Motor Claim Forms

The use of these forms should be read in conjunction with the Motor frequently asked questions (FAQ's).

  • Agency driver to complete
    An agency driver needs to complete this form when one of the agencies registered motor vehicles is involved in an accident. The form can be utilised for all types of motor claims, including:
    • windscreen claims
    • single vehicle accidents
    • multiple vehicle accidents or
    • total losses (i.e. where vehicle is written off)
    In the event an agency vehicle is involved in an accident with another vehicle or causes property damage, where the agency driver is at-fault but there is no damage to the agency vehicle, a claim form is still required. This is so RiskCover may settle the other party’s (ie. Third Party’s) claim.
  • Third party to complete

    This form must be completed by a Third Party in the event that they wish to claim direct off RiskCover when an Agency vehicle has damaged a Third Party vehicle and/or their property.

    The request of this information from the Third Party does not imply liability on RiskCover’s part. The Third Party may not have insurance and/or may not wish to claim direct against his/her own insurance, so the request of this information is to assist RiskCover with claim liability/management to settle the Third Party’s claim.

    This form enables RiskCover to access information to assist with processing a claim. You are not obliged to provide consent, however, failure to consent to the disclosure of information as outlined on the form may delay a decision by RiskCover on your claim.
