Property Claim Form

The use of these forms should be read in conjunction with the Property frequently asked questions (FAQ's).

  • Agency to complete

    This form must be completed by an agency when they suffer loss or damage to property owned by or in the care, custody or control of the agency and wish to lodge a claim.

    The Property claim form must be fully completed and signed along with the relevant supporting documentation (such as a quote, receipt, invoice, asset or resource listing). Please see the Property FAQ's for more information.

  • Employee to complete, Agency to approve

    This form must be completed by an employee of an agency and approved by an authorised agency representative when they suffer loss or damage to property while being transferred to another location in the course of their employment. The employee may effect cover for loss of or damage to their household furniture and personal effects (including vehicles) whilst being transported to the other location or placed into storage during the period of transfer.

    The Property in Transit Claim form must be fully completed and signed along with the relevant supporting documentation (which includes inventory list and repair or replacement quotations). Please see the Employee Property in Transit FAQ's for information on submitting the form.
